- Initial Sign-Up Form: The process begins with an initial sign-up form to quickly check whether you qualify for the medication. You’ll be asked to enter your current and desired weight in pounds. Next, you’ll be asked to indicate your state. This ensures that a state-licensed provider is available and medication can be dispatched to your state.
